Holmdel Township in partnership with Ocean Health Initiatives will be offering a COVID-19 PCR testing site on Friday, January 22, 2021 from 9:00 am - 3:00 pm. Because of the limited number of PCR testing kits, pre-registration is required.

In order to shorten the wait time at the test site, time block sessions will be on an hourly basis. Please register for one of the six time block sessions below by clicking on the REGISTER button to the right of the session. PLEASE ONLY REGISTER FOR ONE TESTING TIME BLOCK SESSION.

The mobile test site will be at the Holmdel Swim Club, 21 Bailey Lane, Holmdel, NJ 07733. The test site will be drive-through and limited contact. 


At the bottom of the registration form you will notice a link to a Pre-Appointment Patient Information Update Form. Please print and complete the form and bring it with you on the test day to expedite the process. Don't forget to submit your registration form. If you do not receive a confirmation email then your registration has not been completed.


IMPORTANT: Each person receiving a PCR test must submit a separate online registration and fill-out a separate Patient Information Update Form.
